Documents required for AQSIQ Application

1. The Electronic Word Version of the Application Form;

1. The notarized business ~and tax registration documents in duplicate,

Of which samples can be provided for reference;

P.S. The documents has to be notarized in the local Notary Public ~or the local Chinese Embassy ~and of which has to present the company name,company address ~and the legal representative of the company,otherwise regarded as invalid documents;

According to China Commissioned Notary (Hong Kong) Management Approach(The Ministry of justice of the people's Republic of China Decree No. 69),applicants in Hong Kong can get all the required documents notarized in Hong Kong Lawyer Office,which we can help the applicants with the notary as well;

For applicants in Japan,the business ~and tax registration documents are required to be originally issued by Legal Affairs Bureau,notarization is not required then;

For applicants in Netherlands,the business ~and tax registration documents are required to be originally issued by Chamber of Commerce in Netherlands,notarization is not required then;

For applicants in Australia,he business ~and tax registration documents are required to be originally issued by Australian Securities ~and Investments Commission,notarization is not required then;

3.Company organization structure, responsibilities of departments ~and job positions,including Purchase,Inspection,Training Dept. Etc;

4. The plan view ~or layout of the office, the warehouse ~and the processing location if you have one,the dimensions (length ~and width) of the all location ~and each departments must be shown on the plan view.

5. The photos ~or video of the office, the warehouse ~and the processing location if you have one. The photos should show us the panoramic view of the company, both inside ~and outside the office building, it would be much better if the pictures can show us the company logo, company name, company address ~and the signpost of the main street,each department of the office which has to conform with the application form,main production line,equipments ~and processing flow path;

6. ISO9001 quality management system certificate, ~or RIOS system certification ~and the operational guidance documents.

If companies do not have the ISO9001 quality management system certification, please provide the company's main business flow path ~and procurement, testing, marketing, training, processing ~and related system regulations, we’ll prepare the operational guidance documents ~and file for ISO 9001 for you.If companies have ISO9001 quality management system certification, ~or RIOS system, please provide the color copy of the certificate ~and related operational guidance documents, including raw materials inspection system, nonconforming product control system, training system ~and processing system (for yard only), the related documents should be both in English ~or Chinese.

P.S. The detailed contact information of the ISO agent has to be provided in order to be authenticated by China AQSIQ Dept.
